Liability Waiver, Medical Authorization, and Media Release
By checking this box, I acknowledge that I am voluntarily registering and/or participating in the 5K/10K Fun Run organized by Keaukaha Canoe Club. I understand that running and participating in outdoor athletic events involves inherent risks, including but not limited to falls, collisions, weather conditions, traffic, uneven terrain, dehydration, heat exhaustion, and other risks that may result in injury, illness, permanent disability, or death.
I certify that I am physically able to participate and voluntarily assume all risks associated with participation in this event. In consideration for being allowed to participate, I hereby release and hold harmless Keaukaha Canoe Club, their officers, directors, volunteers, sponsors, event staff, and affiliated organizations from any and all claims, liabilities, damages, or causes of action arising out of participation in this event.
I authorize event organizers and emergency personnel to obtain medical treatment if a participant becomes injured or unable to communicate during the event and understand that participants are responsible for any medical expenses incurred.
I grant permission for Keaukaha Canoe Club to use photographs or video taken during the event that may include participants’ likenesses for promotional or informational purposes without compensation.
If I am registering multiple participants (such as a team, family, or group), I certify that I have informed all participants of this waiver and that I have the authority to agree to these terms on their behalf. If any participant is a minor, I certify that I am their parent or legal guardian and agree to these terms on their behalf.
